Claims must be reported to Insurance Company within 60 days of date of injury.
This to be done by completing at least the first 2 pages of the claim form and submitting claim form via Club Secretary.
Insurance Company will not entertain claims that are not notified in above manner.
If player has Medical Insurance eg. VHI, Quinn Healthcare or with any other Company you must make a claim with that Insurance Company.
Physiotherapy, Osteopathy, Chiropractic Treatment etc. must be medically prescribed and a maximum of €200 per claim for such treatment will be met by the Insurance Company.
The first €60 of each claim is not covered.
Please retain Official Medical Receipts, these will be required for claim.
The Club has decided to put a cap of €300 on any one claim, in respect of Physiotherapy and related treatments and will not guarantee to meet claims for such treatments in excess of this amount.
Claim Forms can be downloaded from the G.A.A. website www.gaa.ie or can be obtained from the Club Secretary (Contact details as above)
Claim Forms must be returned via the Club Secretary.
